One of the Buch der heiligen Dreifaltigkeit (BdHD) -wiki-
Based on this posting from bi3mw. This seems to be from the Leiden MS VCF29. According to this French web page, the source is Leiden MS. Vossianus Chym. F. 29. (Erfurt, 1522-1526?)

Although if we compare the first displayed page above and these pages, they seem to have a different manuscript quality or scanned differently.
Perhaps they are different copies.
